What services do we offer?

WELSH EYE HEALTH EXAMINATIONS

We offer the Welsh Eye Health Examination for patients who are eligible. This can be for patients who may experience sudden eye problems or ones who may be at a greater risk of eye disease. 

NHS EYE EXAMINATIONS

.You may be eligible for an NHS funded eye examination, please refer to the list below. You are eligible if you:

• Are under 16, or under 19 but still in full-time education.
• Are of 60 years or older.
• Have diabetes or glaucoma. 
• Receive, or partner receives income support, income-based jobseekers allowance or pension credit guarantee.
• Are over 40 and have a parent, sibling or child who has glaucoma.
• Have been advised by an ophthalmologist that you are at risk of glaucoma.
• Are registered blind or partially sighted.
• Are entitled to NHS complex lens voucher.
• Are named on a valid NHS Tax Credit Exemption Certificate or an HC2 voucher*.

*You can apply for an HC2 voucher by filling in an HC1 form ('Are you entitled to help with health costs?').
